To get Extra ordinary leave EOL (for non training purposes)
You have to write an application in the name of secretary primary and secondary health care department for extraordinary leave. State reason for the leave, for example if your parents have health issues, family issues, or any reason you are facing.
Get leave title from account office. It is kind of NOC. They will give a form for unpaid leave and will give you no objection certificate for unpaid leave for the mentioned duration.You will attach that along with the application and go to you CEO. He will approve it and forward it to Secretary health. He will give you typed orders in which he will mention that he is sending enclosed draft for EOL and ask the secretariat to grant the EOL. They will send this to secretariat, to Concerned hospital and one copy to concerning doctor.
You can either take secretary concerning draft directly to secretariat and allow it yourself which is quicker. Or you will let CEO office to send copy to secretariat through post. which takes upto 2 months. Technically your leaves will start when it gets approved from secretariat.

There are few catching points.
1. The account office will resist giving you NOC without bribe.
2. CEO usually ask for bribes to approve and forward the application. They do such stuff through their PAs.
3. You can vanish as soon as you application gets forwarded to secretariat. It is rarely get rejected once account office and CEO approves it.
4. It better to attach anything that strengthen your reason for eol.

For postgraduate, the EOL is given with required documentations (rtmc, results and training orders)
