If you are working in a BHU and RHC you will frequently came across EDO and DHO visiting your centers and different high authority teams coming to you and demanding thing you have no control over.Like asking you to keep washroom floor so clean that they can see their face in it or sometimes asking you to repair windows , chairs and some staff  :P  . In my experience saying "yes sir ,it will b done immediately " is the right answ er.You do not have to do anything.Their intention is not to fix anything.They do not have the means or money to fulfill your demand.So no point in arguing with them. The more you argue, they more arrogant they will get and in the end nothing will happen. How to manage Utility Bill issues at BHUs: It is the responsibility of incharge BHU to submit monthly utility bills from his pocket and then submit original paid bill along with signed stamped contingent bill form to DHO office-Accounts branch to get it back as soon as possible. From jan17 onwards management system have been changed at district level and it have become a headache to get any arrears/bills etc through clerks. Now a days many MOs have to wait for 3 to 4 months t o get these bills passed and they starts to demand their share from these extraordinary amounts, Which are stacked due to mismanagement on all ends. Dealing with seniors at DHQ,THQ and RHC This question came up very often i. Honestly speaking the answer is very diverse. Some seniors (senior MOs ,SMO,APMO) are very nice and some are very bad. When you work at periphery the two major problem u may face with a senior is 1. He or she tries to skip his/her duty and put all the burden on you 2. They wont let you work in OPDs and make you do emergencies and interfere in your medicolegal buisness. Well be blunt in these cases. Now remember that in periphery word "senior" is not the same as in ur teaching hospital. Here you are an MO. Equally responsible for your action.No one will come to your rescue when things starts going south.They usually do not like new young doctors checking patients efficiently because that may result in increased patient numbers in the hospital and their private clinics will b effected. Show cause notices & dealing with Administration of BHU/RHC/THQ/DHQ Many of new doctors are worried about getting a show cause notice.What is a show cause notice? It is a simple letter to you from the administration of a hospital asking you to explain your particular action.And 99% of the time it goes to cold storage after ur response.Mostly they issue such notice to threat the young doctor as you get panic after a showcause notice. Remember that If you have not received a Show cause notice then it means you are working correctly and verbal threats or orders of your superior are just baseless threats.Never respond to a verbal order if you think that it is not fair. If administration do serve you a notice then there is no need to panic.It is routine procedure.Always give proper written response and state the facts .Always save a copy of ur response after diary. I have come across many young baby doctors in peripheries who ask same questions and have same problems with seniors in peripheries ,,, some of them are below 1. SMO ,WSMO APMO at peripheries at all levels tell their junior doctors that they are 18 ,19 scale or as they are incharge (incase of RHCs) so they are exempted from emergency duties or medicolegal cases..... Well that is totally false.whether it is MO,WMO or SMOs,SWMOs or APMO  or APWMOs they are all general cadre doctors.There is no difference in doing duties between them.Difference come when cadre changes ...like if someone is a specialist and i mean a real specialist with a specialized degree then he has specific duty according to its specialty .Otherwise apart from specialists or MS,AMS or DMS all have to perform same duties .A new MO can do OPD duty and an APMO can do emergency duties.There is legally no difference in job description.They just try to manipulate you k...

Dealing with CEO, DC, and other authorities at periphery This requires lots of brain and cool mind. The CEOs are like your saas   😁 . You have to deal with him but you cannot do everything he asks of you and You can't say no or loose temper even though he is asking you to donate your kidneys. So how to deal with this.... The most efficient and time tested method is to say "yes sir. It will be done ASAP " on anything he or any authority figure is saying to you at the time of visi t or in their office. When they are gone forget that "yes sir" and do nothing. They usually forget what they said. When they come again and say "Dr sahib ye kam ebi tak ni hua"... again reapeat the above procedure after saying "sorry". Improving puntuality of staff at BHU If you are posted at BHU, you will find that many of your staff (LHV, Dispensar etc) will come late or they will disappear after 8.15am after marking attendance on biometric.. I am going to suggest few solutions for this but it is only applicable if you yourself is punctual. When you start a BHU, the staff their tells you that you can come late and they will cover for you... this is a trap. It is one of the most common lie they tell you.When you will come late then they will the the 1st to call CEO or DDHO and they will report you. They do not like drs at their centers. Most of them are there for decades and they are not use to doctors at BHU.They will not miss any opurtunity to get you removed. So never trust them. I will suggest that you order a locked box for your biometric machine so that they cannot break it. Keep the keys to yourself. Because of biometric now its not easy to mark attendance the next day. Problems you may face while working in a BHU . 1.    In a BHU the medical officer is incharge of both as a doctor and as administrator. Which means u r responsible for not just patient care but also for the keep and maintenance of the whole building. 2.    You will be responsible for cleanness of the building, repairs of building, bills, tree counts, etc and u are required to be available in BHU 24/7 even after duty hours (if u r given residency in BHU) 3.    The biggest problem you will face is managing medicine record and the staff. Dispenser and LHVs there do not want a medical officer on their head. They are at more freedom when MO is not in charge. So they will try their best to kick you out.
