How to get EOL (Extra Ordinary Leave) for Postgraduate training

How to get EOL (Extra Ordinary Leave) after selection as PGT if you are working as Regular MO/WMO.
No need for NOC from current place of posting, just convey the message directly or through staff of your health facility to higher officials(SMO, MS, DDHO, DHO, CEO etc) that you have been selected as PGT.
Join at your place of selection of PG Course.
Get all your required documents for EOL (List is attached)
Write an application to Secretary Primary and Secondary Healthcare Department requesting him for grant of EOL with effect from your date of joining(mention it) and get it duly forwarded from your CEO Health.
Your can attach receipt of RTMC from CPSP with application untill you receive your RTMC.
If you consider CEO Health office of your district efficient enough that they themselves will post your application with all documents to Secretariat then its your choice otherwise collect your application with forwarding letter from CEO Health office and attach other documents and submit all this in person to Secretariat with some "bribe".( it is not compulsory and upto you)
You also have to submit an Affidavit (specimen attached).
If your application is incomplete they will inform you through writing.(example letter is attached)
Initially you will be granted EOL for 730 days, after which you have to apply for extension.
