Adhoc Renewal
1. Write an application to concerned CEO DHA mentioning aforesaid subject.
2. If you are working in RHC/THQ/DHQ get it signed and recommended from your SMO/MS. If you are working at some BHU under Punjab Govt. then skip this step and follow next step.
3. Fill Evaluation performa (Link given), get it signed by SMO/MS of concerned Hospital.
4. Get you application & Evaluation signed and recommended by concerned DDHO.
5. Submit application to concerned CEO DHA office with attested copies of all educational documents, 1st Orders Copy, Joining Report and Last Pay Slip. (FOR BHU= They will Submit it in DHO Office and DHO will Recommend and sign to CEO then it will get submitted in CEO Office)
6. CEO DHA will issue a Covering Letter.
7. Next there are two options. Either you submit all documents by hand or ask CEO Office Clerk to forward via mail. I will prefer by hand submission in Secretary P&S Health office.
8. Minutes of Meeting Document is very necessary to be attached with it(only for those whose interview was conducted by District Level). This document is not easy to get because CEO Office Clerks will deny to give this document.
9. Get all the documents, Covering Letter and submit in HISDU P&S Health Lhr.
10. After 2-3 days, your orders will get uploaded on P&S Health website.(It may take longer)
