Working in an RHC
There are few points that new medical officer and WMO should know.
1. RHC has the post of SMO (senior medical officer) .If u have a SMO present in your rhc then it makes things easier.The reason is that in a BHU the major issue is running the administration.We are trained to check patients but we are not aware of any administrator work and that is what gives us the major headache.SMO will be the administrator in rhc.
2. Having SMO simplifies your job.Its his responsibility to do all the administrator work and you simply have to follow the roster and check patients.
3. Now i came across with doctors having trouble with their SMO.Well to b honest when you are new to this, you are kind of a ragging tool for the senior
.You should know that SMO is also a medical officer there and is not exempted from duties like an MS.He has to perform his duties as MO and as an Administrator as well.He is kind of a MO in BHU.So if he tries to make you do all the duties or make u do extra hours then you can simply refuse him.Although its better to negotiate with him.Its polite way.You both have to do emergencies and OPD as agreed between you.Recently they have increased seats in RHC.
4. It is SMO responsibility to b Present 24/7 at a RHC and is to reside in the premises .He is responsible for upkeep of medicine, handling of paramedics etc and building.He can assign u a responsibility and u have to do that under his supervision but still he is responsible for those too.You can call him anytime and he has to come for your aid.
5. MLC are responsibility of both MO WMO and SMO .Now postmortems are also authorized at RHC.
6. The WMO is responsible for usually gynae ER and OPD.WMO should not do MLC of males if a male doctor is appointed at rhc including SMO no matter what.simply refuse to perform MLC of males.SMO if push you or threaten u then do not listen.
7. Always remember that Nobody will give you a written order if it is against the rules or he may think that it can b used against him in court of law.This is the most important point.Never do any controversial work on verbal orders. Ask for written orders and if they(Administrators) are fool enough to give you a written order ,for example asking female to perform a male MLC then u can simply challenge that above or in court of law.Usually they do not give anything written.
8. Many are asking about duty hours... technically one male doctor and one female doctor has to b present at RHC all the time for MLCs and emergency case.You technically cant b on call.An mlc has to b declared as soon as docket is available.and any emergency case should b attended ASAP.Now it varies on RHC to RHC. but if ur SMO is insisting u to give night covers then nothing u can do about it.RHC is a referral point so it cannot stay without doctors.

Try not to collide with your senior though.Its a mutual relationship.As doctors we respect our seniors.Mostly they are nice.Though never do anything that will endanger u.Because when inquire is set then nobody will come to your aid.
